Abstract

The fractures of the lateral process of the talus are uncommon and often overlooked on the initial roentgenograms. If overlooked and untreated, these fractures can lead to long-term complications and disability. A retrospective review of seven patients with prompt treatment indicates that this appears to lead to best result. The diagnosis was prompt because the CT scan was used when there was a doubt on the X-rays or in the persistant ankle pain.

Nondisplaced (<2 mm) small fragments should be treated in a short-leg cast, with 6 weeks of immobilization. Large displaced fractures require open reduction and fixation. Comminuted fractures should be excised. Symptomatic ununited fractures should be treated by removal of small fragments and fixation of large fragments. An osteotomy may be performed in case of symptomatic mal-union. Symptomatic subtalar osteoarthritis may require a subtalar arthrodesis.
